Complimentary conference on the Advances in the treatment of long term conditions:
lessons learnt.
In partnership, Graphnet Healthcare and Salford Primary Care Trust have succeeded in developing an integrated diabetes care record (LTCII). The LTCII system incorporates information from both primary and secondary care systems in the region and also allows patients to have access to their complete diabetic care record. Secure access to patient information is available at any time, even when the treatment is delivered by a service that does not hold clinical records.
The success of this system has already been recognised nationally and Salford PCT was highly commended at the 2006 HSJ Awards for 'Improving Care with E-Technology'.
On the 12th March, Salford Primary Care Trust and Graphnet HealthCare will be sharing the lessons learnt about overcoming the challenges of:
Obtaining patient consent
Supporting patient self-efficacy
Ensuring data quality
Winning clinical buy-in
