monitor progress of challenges and get an accurate view of its current annual expenditure

iQ Predictive Health Manager will identify patients at risk of
hospitalisation so that resources can be targeted in order to optimise service delivery,
reduce costs and improve patient outcomes. It automatically extracts electronic
patient records from all GP clinical systems and IP, OP and A&E data from SUS.
Individual practice, PBC consortia or total PCT populations can be stratified to
give a risk profile and by each long term condition so that interventions can be
matched to care needs.

into patients’ lifestyles, behaviours and needs. This is invaluable for tackling
inequalities, finding hidden illness, designing services, devising communications
and developing treatment strategies. The ability to map information adds a whole

data, clinical hotspots can be better targeted, investments prioritised and patient
