Since 1994 Graphnet has helped caring professions and related organisations modernise

care record delivering a holistic view of the care process.
Graphnet solutions are based on proven technology that enables the interoperability
of information across a natural community such as GP systems, social services, mental
health, secondary and tertiary care systems. Information is aggregated from existing
systems across the community to present a single, integrated record at the point
of care. The result is a web-based solution that provides a single portal view to
support care across all of the caring professions.
This data, collected as part of the caring process, can also be analysed to determine
the safest and most appropriate pathways, which deliver improved care.
The record can be enriched with additional information and functionality from programs
provided by Graphnet, internal development teams, legacy systems or third-parties
without the need to 'rip and replace' existing systems or infrastructure.
The modular structure of Care-Centric allows components to be introduced incrementally
to meet the specific needs and priorities of individual organisations.
