Challenge

The care co-ordination of service users with mental health problems requires involvement across health organisations, such as mental health, general practice, acute trusts and specialist services. Many health and social care professionals now work as integrated teams resulting in the need for coordination with social care organisations and other care services such as the police and voluntary support organisations.

Care for service users with mental health problems is provided in a wide range of settings. Care professionals must be able to access service user records regardless of their location; whether it is a secure unit or in the service user's home including remote locations such as courts, prisons etc where service user contact may occur.

Solution

The Graphnet Mental Health system can meet all these challenges. It gives care professionals access to the right information they need at the point of care. The system pulls together data from disparate systems across the health community to provide a single, service user centric record. This web based record can then be accessed and added to by care professionals involved in the service users care. This integrated record enables efficient cross agency working and provides a coordinated and seamless provision of service for service users.

Within strict roles based access, the Graphnet Mental Health system provides care professionals with the ability to view current medications, develop care plans; undertake risk assessments; section and share relevant information with others involved in the service users care. There is also the ability to produce reports and analyse data to fulfil reporting requirements.

The advanced messaging capabilities of the Graphnet system enables integration with legacy systems including CfH PAS systems to allow administration information such as ward location for inpatients, Mental Health Act status and future appointments to be viewable and updateable from a single portal. Secure messaging allows transmission of information safely, protecting confidentiality.
