Integrating London’s child health records: safer and better care in just four months.

The System C & Graphnet Care Alliance has worked in partnership with London’s commissioned services to replace 18 separate child record systems across 32 boroughs with a single pan-London electronic patient record solution, in record time. As a result of the new London Child Information Service (CHIS), administration time has halved in one area and comprehensive data is available across a whole region in hours, not weeks. Within a few hours of being requested by the Grenfell Tower action team, the CHIS produced a full list of every child who had been resident in the previous six months.

Adding Out of Hours services to Care and Health Information Exchange (formerly Hampshire Health Record)

The Care and Health Information Exchange (CHIE) has transformed how patient information is shared between health and social care services across Hampshire, Isle of Wight, Farnham and surrounding borders.

For example, that wealth of information is available to Out of Hours (OOH) services across Hampshire, who use it to support urgent clinical decision-making outside of normal hours.

Digital Shared Care Records in Context: The National Strategy

Shared care record systems occupy a central position in today’s vision of a transformed NHS fit for the 21st century.

Wolverhampton CCG’s diabetes care management system
Solutions for targeted groups of patients

Wolverhampton CCG is using Graphnet’s CareCentric product suite to deliver a diabetes management system for the 16,500 people in its area with diabetes.

CareCentric at Poole Hospital NHS Foundation Trust
Solutions for acute care

Poole Hospital NHS Foundation Trust and Graphnet have been working in partnership for 13 years. Over that time, Poole has successfully used the CareCentric product suite to implement an electronic patient record system which is tailored specifically to meet the Trust’s needs, working practices and pathways. The system is now used by over 5,000 users at the hospital itself and in clinical networks, GP practices and social services across Dorset.

The Hampshire Health Record
Solutions for whole health communities

The Hampshire Health Record (HHR) is a shared care record which provides a single source of patient information pulled from a multitude of different systems right across Hampshire. It is routinely used by clinicians to share information about nearly two million patients and to support clinical decision-making.

Analytics services
Reporting and business intelligence solutions

The Hampshire Health Record analytics service (HHRa) provides clinical and administrative reporting services using data collated from the majority of clinical systems in use across Hampshire. Using pseudonymised records extracted from the Hampshire Health Record (HHR), HHRa is able to provide a wealth of regular and ad hoc reports in support of clinical decision-making.
