By providing a single up-to-date record which can be shared and updated across a whole health community, CareCentric allows healthcare commissioners and multi-disciplinary teams to plan, develop and deliver integrated care services.
This is particularly useful for patients identified as being at risk of unscheduled or unplanned care, for those with long term conditions such as diabetes, or for frail older people.
CareCentric can be used to develop a single integrated care plan for identified patients or groups of patients. All authorised professionals involved in that patient’s care, including community-based services, then have immediate, on-the-spot access to that patient’s records and care plans. The plan is updated in real time, and so provides a single up-to-date record.
This care planning process is an important step in providing more proactive services to a local community. It helps involve patients in managing and making decisions about their own health. It also helps care services ensure they collaborate closely with each other and provide more responsive, effective and timely care.
