WEBINAR: Joining the health & social care dots with data

Population Health is expected to play a crucial role in the Government’s vision in delivering joined-up services via Integrated Care Systems (ICSs) in England. The analysis of data at a regional level enables organisations to intervene earlier with targeted action for people who show signs of risk or deterioration.

One cohort that this data can be used to support is older people and those with complex needs. Targeted pro-active support has the potential to reduce hospital visits and enable people to remain at home and independent for longer.

Graphnet would like to invite you to an event with our partner Liquidlogic which looks at how data can be used by integrated care systems in preventative services. This event will feature presentations and demonstrations across the social care and health sector. Speakers include:

  • Martin Farran, former Director of Adult Services & Health at Liverpool, York and Barnsley councils to speak about the use of data in early intervention.
  • Jim Hughes, Strategic Advisor Digital Programmes and CIPHA Programme SRO will discuss the CIPHA project which combines local and national data feeds in real-time, presenting information in the population health platform to health and social care teams.
  • Sharon Boundy, Associate Director System Transformation, and Mark Sellman, CIO, from Frimley Health and Care ICS will share how health and care professionals are using applied population health to identify residents who could benefit from proactive care.
