According to the Child Poverty Action Group[1], over half of households in the UK (15 million) are in fuel poverty. This is defined as spending over 10 per cent of net income on fuel.
Fuel poverty can exacerbate symptoms for those with long-term health conditions such as cardiovascular and respiratory disease, mental health issues or a disability. Using population health management tools, the health community is equipped with the data to identify those who are most severely affected in order to support people to achieve better health outcomes.
We would like to invite you to join a webinar, where you will hear from Dr Priya Kumar, GP Partner Kumar Medical Centre, Health Inequalities Lead, Slough, Connected Care Clinical Lead and
Dr Lesley Rosling, GP Principal at The Cambridge Practice on how data within the shared care record is being used to identify vulnerable patients impacted by socio-economic challenges to proactively support their community, drive action, and actively tackle reducing health inequalities.
