QOF: segmentation for better outcomes

The Quality & Outcomes Framework (QOF) demands that patients with specific conditions are reviewed on an annual basis. Increasing demands on GP surgeries can mean that QOF reviews, although essential, are carried out in a non-targeted way with patients who pose a greater risk often in the queue behind those with less urgent needs. But how can already overstretched surgeries take time to segment more complex patients as a priority?

Using the Graphnet Population Health platform, Frimley Health & Care applied the Johns Hopkins ACG PNG risk stratification tool to proactively segment patients based on 11 needs groups. This led to those with more complex being seen by an appropriate member of staff early in the year and prior to seasonal pressures taking effect. This has meant a reduction in repeated surgery visits and enables practices to support complex patients sooner, meaning there are less chances of those patients deteriorating further and requiring wider NHS support.

QOF segmentation has:

·        Freed up medical staff through winter with priority cases seen prior to this period

·        Ensured that patients with more complex needs are proactively reviewed earlier

·        Led to a reduction in appointments taken because the appropriate member of staff is seen in the first instance

·        Maximised QOF points and therefore, earnings

Frimley’s holistic approach means that appropriate resources have been directed to patients who are most in need, leading to better management of known conditions with patients being segmented and prioritised based on several indicators across a range of key areas of clinical care and public health.
