As a key partner, Graphnet is delighted to learn that the Connected Care programme in Berkshire has been shortlisted for the Enhancing Care by Sharing Data and Information category at the 2017 Health Service Journal (HSJ) Awards.
This prestigious award recognises the delivery of the Connected Care programme by Frimley Accountable Care System, West Berkshire Accountable Care System, and South Central and West Commissioning Support Unit.
Connected Care, built using Graphnet's CareCentric software, is providing integrated data solutions for 18 health and social care partners across Berkshire. The programme sees the design of an interoperability solution which provides unique opportunities for health and social care providers to access a whole system approach to data analytics.
Not only is this announcement recognition of the achievements of Connected Care and the difference it makes to health and care professionals and patients, it is also illustrates the growth of digital services at the heart of the NHS.
Members of the Connected Care team will now produce presentations and be interviewed by a judging panel made up of senior and influential figures from the health sector. The awards ceremony will be held on 22 November at the Intercontinental 02 in London.
