Graphnet and its partners in the Epilepsy Care Alliance (Poole Hospital, University of Kent and Shearwater Systems) are celebrating being shortlisted as finalists in this year’s HSJ Awards under the Supported Self Care category.
The consortium developed the myCareCentric Epilepsy app to empower adults with epilepsy to take a more active role in their health and care. Integrating the patient’s shared care record with smartphones, wearables, clinical communications and machine learning, the app is an end-to-end solution that provides support at the time of need as close as possible to the patient and in their hand.

To be shortlisted as a finalist in the Supported Self Care Category of the HSJ awards, the Epilepsy Care Alliance had to demonstrate how it is supporting patients to develop the knowledge, skills and confidence to improve their own health and simultaneously boosting outcomes, experience and efficiencies.
Winners of the HSJ Awards will be announced at an awards ceremony on 22 November 2017 in London.
