Manchester Care Record shortlisted for innovation award

The Manchester Care Record has been shortlisted for a North West Informatics Awards 2015 for innovation.
The project combines an integrated care record with a care planning application. It is designed to help multi-disciplinary teams protect the city’s most vulnerable people and dramatically improve care outcomes.
Graphnet’s CareCentric software brings together information from 3 CCGs, 3 acute Trusts, Manchester City Council’s social services department and 90 GP practices. It gives authorised professionals immediate, on-the-spot access to the records of patients identified as being at risk of unscheduled or unplanned care.
Multi-disciplinary teams have so far developed 6,000 individual care plans and are working together to keep patients healthy and out of hospital.
Early evaluation of the project shows a significant reduction in secondary care activity for people identified as being at high, or very high, risk of hospital admission.
The award winners will be announced at the CONNECT 2015 conference held in Blackpool on Wednesday 16th September.
