St Helens Cares and Graphnet Health are delighted to announce that St Helens Shared Care Record has been shortlisted in the System and Data Integration category at the HSJ Partnership Awards 2020, recognising their outstanding dedication to improving healthcare.
The judging panel comprised a diverse range of highly regarded figures across the NHS and wider healthcare sector. To be shortlisted as a finalist for these awards, despite tough competition from hundreds of brilliant applicants, is a mark of real achievement. St Helens Cares and Graphnet have been selected based on their diligence, ambition and the positive impact that their project has had on both practitioners and patients within the health care industry.
Powered by Graphnet, the St Helens Shared Care Record (SHSCR) unites health and social care data on the borough’s c.178,500 residents into one single system which is accessed by organisations across the place-based St Helens Cares partnership. Before the shared care record, staff had to send emails or make calls whereas the SHSCR presents information directly to care professionals.
The ability to share comprehensive information across organisations is producing many benefits in terms of safety, efficiency and quality of care including:
Supporting efforts to reduce incidents of avoidable harm
Reducing unnecessary admission and minimise stay
Avoiding unnecessary/incorrect testing, diagnosis and treatment
Reducing staff time spent hunting and gathering
Reducing the need for a person to tell/retell their story
Members of the SHSCR team will now produce presentations and be interviewed by a judging panel made up of senior and influential figures from the health sector. Winners of the HSJ Partnership Awards will be announced at an awards ceremony on 27 February 2020 in London.
