The GMCR brings together data from multiple health and care disciplines, giving professionals access to vital information to:

• Deliver timely, targeted interventions
• Track impact and outcomes over time

We’ll also explore how integrated care and transformed public services can deliver solutions that go beyond medicine alone, and the significant role technology plays in addressing health disparities and improving health equity.

By combining data from multiple health and social care sources into a single Shared Care Record, care teams can gain a holistic view of people and populations, enabling more proactive, preventative, and personalised care.
