Frailty Doesn’t Have to Mean Crisis: Moving from Reactive to Preventative Care webinar

24th November 2026 , 11:00 to 12:00

Tuesday 24 November 2026 | 11:00–12:00 GMT | Webinar

Discover how South Liverpool is using population health intelligence and multidisciplinary neighbourhood teams to strengthen frailty care.

What if we could spot frailty earlier, intervene proactively, and help more older people live independently at home?

As the NHS shifts towards preventative neighbourhood care, SWAGGA PCN is leading the way by showcasing how collaboration among healthcare professionals can transform frailty services.

Join Luke Wilkinson, GP lead for the SWAGGA PCN Ageing Well Hub at South Liverpool Treatment Centre, to hear about the Hub’s work in collaboration with LUHFT.

Luke will share experiences and practical learning from a service that brings together primary care, community services and specialist expertise to identify people at risk, carry out comprehensive geriatric assessments (CGAs) and coordinate personalised care closer to home.

What you’ll learn

  • How CIPHA population health intelligence supports early identification of frailty and risk stratification.

  • How multidisciplinary teams work across services to provide holistic assessments.

  • How personalised care planning aims to support independence and prevent deterioration.

  • What early patient experiences and practical learning can tell us about developing neighbourhood frailty care.

We’ll also explore plans to support housebound patients and strengthen links between neighbourhood and hospital frailty teams.

Why this matters

According to the National Audit Office, frailty affects at least 1.5 million people in England aged 65 and over.¹

Frailty is already a significant challenge for health and care services, and as our population ages, the need for earlier identification and proactive, coordinated care will only grow.
