Living & Ageing Well webinar: Better decisions, better outcomes

10th March 2026 , 10:00 to 11:15

The NHS is under growing pressure from an ageing population, with people aged 65+ projected to make up around a quarter of the UK population within the next 20 years [1]. At the same time, demand from people living with long-term conditions already accounts for around 70% of health and social care spending [2], while too many older people experience fragmented care, long waits, and avoidable hospital stays. Joined-up, person-centered care planning is no longer optional, it is critical to supporting people to live and age well, while helping the system work more effectively.

Join us on 10 March, 10:00–11:15 for our latest webinar, Living and Ageing Well, where customers share real-world experiences of using digital care planning to improve outcomes for older people.

Discover how the Comprehensive Geriatric Assessment (CGA), EPaCCS and ReSPECT are being used in practice to enable joined-up, person-centred care, improving collaboration across teams and supporting individuals to be actively involved in their own care. We will also explore how our digital Care Planning suite integrates with the Graphnet Shared Care Record to provide a single, real-time view of care plans - supporting professionals, patients and carers alike.

We are delighted to welcome expert speakers from across health and care, who will share insights on:
•    CGA (Comprehensive Geriatric Assessment)
Dr Joanna Seeley, Specialist Community Geriatrician, East Kent Urgent Care Services
Explore how digitised CGA brings together medical, functional, psychological, and social information to enable coordinated, person-centred care.
•    EPaCCS (Electronic Palliative Care Coordination Systems)
Dr Dave Waterman, Consultant in Palliative Medicine and End of Life Care Clinical Lead for Greater Manchester and Eastern Cheshire
Learn how a unified digital approach supports high-quality palliative and end-of-life care across health and social care.
•    ReSPECT (Recommended Summary Plan for Emergency Care and Treatment)
Matthew Hutton, Integrated Care Digital Lead, Integrated Care Northamptonshire
See how ReSPECT empowers individuals to record their preferences and ensures their wishes are respected in emergency situations.

Who should attend?
Clinicians, care professionals, system leaders and digital transformation teams – any health and care colleagues involved in enhancing the quality of life for patients, improving outcomes for individuals with long-term conditions, complex needs, or those considering end of life care planning.
