Neighbourhood Health: From Strategy to Reality

Neighbourhood health is a key enabler of the NHS 10 Year Plan’s three strategic shifts and technology is expected to play a crucial role in its' success.

  1. Moving care from hospitals into communities
  2. Switching from analogue to digital
  3. From sickness to prevention. 

As a result, it is becoming a central focus as the NHS moves towards more preventative, proactive models of care delivered closer to home. ICBs are increasingly expected to use joined-up data, population health management and digital innovation to identify people earlier, reduce pressure on acute services and enable neighbourhood teams to deliver more coordinated, personalised care within local communities.
