Partnership Set to Transform NHS Care for 500,000 Patients
Graphnet Health and Luscii, an OMRON Healthcare Service have announced a landmark UK partnership, launching Graphnet Remote Monitoring (Graphnet RM) - the most advanced remote monitoring platform of its kind. The first trusts have been enrolled with Graphnet RM becoming available for nurses and doctors to treat their patients remotely through the app and advanced algorithms. More trusts are already scheduled to follow.
The move comes in response to rapid developments in preventative and digitally enabled out-of-hospital care, with forecasts suggesting an additional 500,000 patients will join remote monitoring programmes in the next two years.
Together, Graphnet and Luscii bring unparalleled reach and scale to the NHS. The platform is already embedded in 20 NHS Integrated Care Systems, serving 17 million people through Population Health and Shared Care Record services, and has delivered remote monitoring to more than 75,000 NHS patients and over 400,000 across Europe, with over 60 million patient days monitored.
Ranked No. 1 in Europe and active in 11 countries, the partnership is further strengthened by Luscii’s parent company OMRON Healthcare and their global device infrastructure, with 350 million blood pressure monitors sold worldwide and over 2 million supplied in the UK in just three years.
Graphnet and Luscii provide the only fully integrated solution combining Population Health, Shared Care Records and Remote Monitoring. This enables the NHS to identify patients suitable for remote monitoring, deliver proactive out-of-hospital care, evaluate outcomes in real time and access advanced population-level insights - optimising capacity and delivering measurable financial impact.
This combined capability will deliver exponential growth in remote monitoring services, directly supporting the ambitions of the NHS Long Term Plan.

Integrated population health and remote monitoring programmes are already doubling improvements in patient outcomes for remotely managed patients. Evaluations show monitored patients experience up to 50% reductions in hospital admissions and A&E attendances.
