Graphnet Health wins landmark Jersey contract to deliver first island-wide remote monitoring service

Family Nursing & Home Care and Digital Jersey select Graphnet Remote Monitoring to support frailty, falls, diabetes and wound care.
Graphnet Health has been selected by Jersey’s Family Nursing & Home Care (FNHC) to provide the island’s first large-scale remote monitoring service, in a landmark project funded through the Impact Jersey CareTech Challenge programme and delivered by Digital Jersey.
The contract will see FNHC use Graphnet’s Remote Monitoring solution, powered by Luscii, to track the health of frail and vulnerable islanders at home and in community settings. The initiative is part of an almost £800,000 CareTech grant awarded to FNHC to roll out a digital monitoring system for older residents in Jersey.
Jersey is a self-governing British Crown Dependency with full responsibility for its own health and public health services. This deployment represents Graphnet’s first island community implementation of its remote monitoring platform, which is already used across NHS integrated care systems in England to help reduce pressure on hospitals and support care at home.
Supporting more independent living for older islanders
FNHC is Jersey’s registered provider of community health care services for adult and children providing expert care and support for people in their own homes and communities.
Under the new contract:
· Year 1 (from January 2026) will focus on frailty and falls, supporting 100 people with remote monitoring at any one time on a rolling caseload.
· Year 2 will add pathways for diabetes and wound management, supporting a further 100 people at any one time.
Using the Graphnet Remote Monitoring app and connected devices, patients and carers will be able to record key observations at home, such as blood pressure, weight and wound images. FNHC clinicians will see this information in near real time and receive alerts if readings fall outside agreed parameters, so they can intervene earlier if someone’s health begins to decline.
Evidence from similar programmes in England has shown that combining remote monitoring with population health and shared care records can reduce emergency hospital admissions and A&E attendances by up to 50 per cent for some patient cohorts.
Putting community nursing at the centre of digital care

This project gives our teams a powerful new way to keep a closer eye on people in their own homes, without adding unnecessary burden to patients or staff. By using Graphnet’s remote monitoring technology, supported by the CareTech Challenge funding, we will be able to spot early warning signs, intervene sooner and help more islanders remain independent, safe and well for longer.

Graphnet solutions support more than 20 million residents, with remote monitoring used in multiple integrated care systems and care settings.
FNHC, Digital Jersey and Graphnet are also exploring plans for local engagement events and demonstrations to showcase how the new service works for patients, carers and professionals.
