Johns Hopkins Healthcare Solutions

Johns Hopkins HealthCare Solutions

Johns Hopkins HealthCare Solutions is pleased to be able to collaborate with Graphnet to support the roll out of the Johns Hopkins ACG® System across the NHS. The ACG System will have a role to play in supporting the Population Health Management agenda and the work of Integrated Care Systems. The integration of the ACG System into Graphnet’s CareCentric PHM platform will help support the work of clinical teams through the accurate identification of the right people for the right care management intervention. It will also assist ICSs in their strategic planning through a more granular understanding of how risk and disease prevalence is distributed within their populations and to support a more sophisticated approach to allocating resources to reflect need and reduce inequity.

Microsoft

Microsoft

Graphnet is working with Microsoft on solutions that use cloud technology and machine learning to further transform health and social care and enhance user experience. Cloud provides a secure, fully mobile environment and a more open framework for interoperability between systems. The adoption to a cloud and internet-first strategy means we are aligning our customers with the NHS’s agenda and helping with delivering the services patients and clinicians need and expect.
