Supporting Enhanced Care in Care Homes

For years, Graphnet has actively supported the improvement of care standards in care homes, especially under the Enhanced Health in Care Homes (EHCH) programme.By aligning with EHCH standards, DOC@HOME is tailored to meet the unique needs of care homes, allowing clinicians to monitor all residents’ health and establish personalised baselines.
This approach benefits residents with chronic conditions while also simplifying the recording of health observations, regardless of underlying health issues.
The result: significant time savings for both GPs and carers, ultimately benefiting the NHS.

Empowering Patients and Saving Resources

DOC@HOME not only conserves NHS resources but also equips individuals to take charge of their health, fostering timely awareness and response to medical concerns.

The virtual wards programme in Staffordshire has been transformative in supporting our aspirations to deliver care closer to home. The ability to remotely monitor people enables us to safely and efficiently manage our patients. This model of care not only enhances clinical outcomes but also empowers our patients to engage with their health needs from the comfort of their own homes, whilst they are being treated for acute illnesses. The consultants in the hospital can access patient data real-time from the Docobo Remote Monitoring system through our integrated care records.

How does DOC@HOME Work?

DOC@HOME gathers data through a range of patient access devices, including tablets, smartphones, web portals, and messaging. Patients can use the DOCOBOAPP™ on their personal Android or iOS devices or opt for a pre-configured device with software pre-installed for a streamlined, user-friendly experience.

Additionally, patients can access DOC@HOME via DOCOBOWEB™, compatible with any device that runs standard web browsers such as Chrome, Safari, or Firefox. This flexibility ensures accessibility for all patients, preventing digital exclusion.

All devices link to the central DOC@HOME clinical system—an accredited medical device—providing secure, real-time monitoring that supports better outcomes and enhances the patient experience.
