As part of our webinar series, we are delighted to confirm Andrea Astbury, Deputy Director of Strategy & Integration (Liverpool Place) and DIA Programme Director, and Chris Buckels, Deputy Head of Business Intelligence (Population Health into Action) from Cheshire and Merseyside ICB, who will be speaking about the innovative work of the Data into Action programme that uses Population Health Analytics to proactively identify the most complex patients across their region.
The Cheshire and Merseyside Integrated Care Partnership serves over 2.7 million people and faces significant health inequalities in the region, including high rates of long-term conditions, mental health issues, and mortality in deprived areas.
The Data into Action programme represents the partnership’s approach to leveraging linked, population-level data. It aims to enable partner organisations to be more proactive, identify patients with the most complex needs, and evaluate the effectiveness of interventions through shared learning.
This webinar will provide an overview of the principles underpinning the strategy and examples of how case finding has been used to identify suitable patients for telehealth, Integrated Care Teams, and multidisciplinary teams (MDTs). It will also highlight the Population Health Academy’s role in building a data-driven culture for action.
