Graphnet partner awarded highly commended at HSJ Awards

The St Helens Cares partnership won highly commended in last night’s prestigious HSJ Awards under the Health and Local Government Partnership Award category. St Helens Cares uses Graphnet’s shared care record to underpin joint working by bringing together health and social care information.

Judges were looking for evidence of strong, collaborative partnerships which were driving improvements for patients and citizens.  St Helens Cares, the local care system was chosen for its outstanding contribution to integrated health and social care from more than 1,000 entries.

The key to embedding integrated working across St Helens has been the roll out of Graphnet’s shared care record that unites resident’s health and social care information into one single system which is accessed by all partner organisations across the region. 

Social care teams at the council, staff at St Helens and Knowsley Teaching Hospitals NHS Trust, GPs, community and mental health workers and other teams are sharing health and care records on the borough’s 178,500 residents meaning they can see the ‘whole picture’ of relevant information for each individual they care for.
